What Are Sesamoid Injuries?
Sesamoid injuries affect the two small, pea-shaped bones located beneath the big toe joint at the ball of the foot. These bones, known as the sesamoids, are embedded within the tendons that help the big toe move and play an important role in absorbing weight-bearing forces during walking, running, and jumping.
Because the sesamoid bones support the foot with every step, they are exposed to significant stress. Repetitive pressure, overuse, or a sudden injury can damage the bones or the surrounding tendons, resulting in pain and reduced mobility. Sesamoid injuries are common among athletes, dancers, runners, and individuals whose activities place repeated stress on the front of the foot. However, they can also occur during everyday activities or after an isolated traumatic event.
Sesamoid injuries can range from inflammation of the surrounding tendon (sesamoiditis) to stress fractures, acute fractures, or displacement of the sesamoid bones.
What Are the Causes of Sesamoid Injuries?
Sesamoid injuries develop when excessive force or repetitive stress is placed on the ball of the foot. Common causes include:
- Repetitive high-impact activities such as running, basketball, tennis, or dancing
- Sudden trauma, such as landing heavily on the forefoot or dropping a heavy object on the foot
- Overuse from prolonged standing or walking on hard surfaces
- Wearing high heels or footwear that places excessive pressure on the front of the foot
- Foot structure abnormalities, including high arches or an unusually high-arched gait
- Poor-fitting shoes that fail to provide adequate cushioning or support
- Rapid increases in training intensity or exercise volume
- Degenerative changes or underlying arthritis affecting the big toe joint
Certain individuals, particularly athletes and those involved in activities requiring frequent push-off movements, may have a greater risk of developing sesamoid injuries.
What Are the Symptoms of Sesamoid Injuries?
Symptoms may develop gradually or occur suddenly, depending on the type of injury. Common signs and symptoms include:
- Pain beneath the big toe at the ball of the foot
- Pain that worsens with walking, running, or pushing off the toes
- Swelling around the base of the big toe
- Bruising following an acute injury
- Difficulty bending or straightening the big toe
- Tenderness when pressure is applied beneath the joint
- Difficulty bearing weight on the affected foot
- A feeling of weakness or instability during movement
The severity of symptoms varies depending on whether the injury involves inflammation, a stress fracture, or a complete fracture.
How Are Sesamoid Injuries Diagnosed?
Diagnosis begins with a detailed medical history and physical examination. Your provider will evaluate the location of your pain, assess the range of motion of the big toe, and identify areas of tenderness or swelling.
Imaging studies are often recommended to confirm the diagnosis and determine the severity of the injury. X-rays can identify fractures or changes in the sesamoid bones, although stress fractures may not be immediately visible. If additional detail is needed, advanced imaging such as MRI, CT scan, or bone scan may be used to detect stress injuries, inflammation, tendon damage, or subtle fractures that cannot be seen on standard X-rays.
An accurate diagnosis helps distinguish sesamoid injuries from other conditions affecting the forefoot, including turf toe, arthritis, tendon injuries, or nerve disorders.
What Are the Treatment Options for Sesamoid Injuries?
Treatment depends on the type and severity of the injury, as well as the patient's activity level and overall health. In many cases, conservative treatment successfully relieves symptoms and allows healing.
Treatment options may include:
- Rest and temporary activity modification
- Ice therapy to reduce pain and inflammation
- Anti-inflammatory medications when appropriate
- Wearing stiff-soled or supportive footwear
- Custom orthotics or padding to reduce pressure beneath the sesamoids
- Taping or immobilization with a walking boot for fractures or more significant injuries
- Physical therapy to improve strength, flexibility, and foot mechanics
- Gradual return to sports and physical activities after healing
Surgery is typically reserved for persistent pain, displaced fractures, nonunion fractures, or cases that do not respond to conservative treatment. Surgical procedures may involve repairing the injured structures or, in select cases, removing one of the sesamoid bones.
Early diagnosis and appropriate treatment can help relieve pain, restore normal foot function, and reduce the risk of long-term complications, allowing most patients to safely return to their daily activities and sports.
